Ingredient & Packaging Suppliers

To our valued current ingredient and packaging suppliers as well as prospective partners, please note the following documentation is necessary to do business with American Licorice Company:

Please Note: By filling out or providing documentation as a prospective partner, your information will be put into a que. Should we pursue any new suppliers, a member of our Procurement Team will reach out for more information using the contact information provided. Until then, your information will remain on file for any of our future needs.

  1. Current Commodity Guarantee form
  2. All Specifications/Product Data Sheets, Allergen Statements, Ingredient List, Nutritional Information, and Country of Origin Statement for all items supplied to American Licorice Company
  3. Certification of compliance with FDA standards for food use and direct food contact as applicable, as well as Good Manufacturing Practices (GMP’s) & USDA/FDA’s Hazard Analysis and Critical Control Point (HACCP) principle
  4. CA Transparency Act Statement or review and signoff on Social Compliance Requirements
  5. Current Certificate of Liability Insurance form
  6. Current copies of Third Party Audit Certificates, Audit Report and Corrective Action
  7. Confirmation of registration with the FDA under the Bioterrorism Act of 2002 and in good standing with the FDA regarding the biennial registration of food facility requirements.
  8. Please take note of our standard PO Terms and Conditions and AAP Notice.

If you are a current partner, you will receive an email from Courtney Schoof with a request for updated copies of the aforementioned documentation for our records. If any questions arise, please either contact Courtney Schoof directly or your Supply Chain Procurement contact.

  1. Interstate Commerce Commission (ICC)Permit/ICC certificate of Public Convenience and Necessity
  2. National Motor Freight Traffic Association Inc. (NMFTA) Certificate of Standard Alpha Code (SCAC) Renewal
  3. Certificate of Liability Insurance
  4. Review and signoff on Social Compliance Requirements
  5. Form W-9/Taxpayer Identification Number and Certification
  6. Hazardous Material Certification of Registration
  7. Any other pertinent certification or documentation you have as a logistics or distribution partner (i.e. United States Environmental Protection Agency Smartway Transport Partner certification, C-TPAT certification, or any other State licenses)
